Inspired by Homero Martinez, who dedicated his life to music, education, and community service, and by Rodney Lee Martinez, a tireless humanitarian leader, our foundation was born from a mission to bring hope and advanced care to those facing heart failure in rural and underserved communities

Our Mission

We are dedicated to transforming the lives of individuals and families affected by heart failure in rural and underserved communities.  Today, more than 6.7 million Americans are living with heart failure, a number that continues to rise. By 2030, it is projected that over 9 million people will be affected. In rural communities, the challenges are even greater, leading to a higher risk of severe outcomes and limited access to advanced care.
Our foundation works to close these gaps by combining community support, advocacy, collaboration with healthcare providers, and innovative solutions. Together, we aim to ensure that no one is left behind—no matter where they live, one heart one community at a time.

Our foundation is committed to addressing these disparities, advocating for those who face not just the challenges of heart failure, but also the obstacles of limited access to advanced treatment and resources.
