Community partnerships in rural and border areas help people understand their condition, access the
right care on time, and feel supported (English/Español).

What is HF?
What are its common signs/symptoms? When should you seek help?
Stages (A–D) NYHA classes (I–IV) Intermacs Scores (1-7)
Ischemic: coronary artery disease, prior heart attack.
Hypertensive & Metabolic: long-standing high blood pressure, diabetes, obesity.
Rhythm-Related: atrial fibrillation and other arrhythmias.
Valve & Structural: aortic/mitral valve disease, cardiomyopathies, structural/congenital heart
disease.
Inflammatory/Infiltrative/Genetic: viral myocarditis, amyloidosis, sarcoidosis, inherited
cardiomyopathies.
Sleep Hormonal: obstructive sleep apnea, thyroid disorders.
Other Contributors: chemotherapy or alcohol-related cardiomyopathy, pregnancy-related
cardiomyopathy.
Daily tools: weight/symptom logs, low-sodium tips, fluid guidance, activity pacing.
Medication basics: what they do, common side effects, red-flag action plan.
